A Rare Case of a Direct Incarcerated Inguinal Hernia Containing an Epiploic Appendage and a Literature Review

Inguinal hernias are a widespread condition, responsible for large number of acute abdomen cases. Typically, indirect, rather than direct, the ones leading to complications, as consequence their narrower hernial defect. Our case concerns rare incidence direct incarcerated hernia in 71-year-old man who presented with pain left inguinal area. Upon clinical examination, an irreducible mass was palpated. Therefore, existence complicated suspected. The patient underwent emergency repair, during which it established that and its sac contained ischemic epiploic appendage. successfully repaired mesh, recovered uneventfully discharged five days later. Despite rarity hernias, they should always be included differential diagnosis groin masses, because can increase severe complications.
